Quick Guide to CSP for Families
Coordinated Service Planning (CSP) focuses on making it easier for families with children and youth with multiple and/or complex special needs to connect with the right services and support based on their strengths and needs.
Coordinated Service Planning is for you and your family if:
- you have a child or youth with complex special needs and;
- you are waiting for or receiving services from more than one agency and;
- you need support to navigate the system
Surrey Place, the lead agency of Coordinated Service Planning in Toronto, and the following agencies; Community Living Toronto, Family Service Toronto, Toronto Central LHIN and Youthlink, work together to deliver this service. This means:

You won’t have to tell your story over and over

You and your child are the most important part of the team

Team members can share information in a secure and confidential way

You will know what services and supports are in your community

Smoother transitions such as into school or moving to adulthood
Families can make referrals by calling 1-833-575-KIDS (5437)
If you agree, a worker may send a referral for you.
